Patient Information

Your First Visit

Please arrive 10-15 minutes prior to your appointment to fill out the proper paperwork. If possible, please fill out your forms here prior to your appointment and bring them with you. Also, bring your insurance information and doctor referral with you (if applicable). Bring in any additional reports as in X-rays, MRI, etc or doctors notes that you may already have. Please wear comfortable clothes. We do have gym clothes available in the clinic if needed.

The initial evaluation will last 1 hour. All evaluations are one on one with your therapist. During that hour, your therapist will assess your condition and discuss patient oriented goals that will direct your treatment. You should walk out of the clinic with a better understanding of your condition and a treatment plan to help you work towards the established goals.

Subsequent Visits

The length of your following visits will be at the discretion of your therapist, based on your needs as a patient. The therapist will actively educate you during treatment sessions to give you a better understanding of your condition. A home exercise program will also be given to you which requires your active participation outside of the clinic.

Cancellation and No Show Policy

A 24 hour cancellation notice is greatly appreciated in order to allow another patient to fill the slot. Arctic Physical Therapy Services reserves the right to charge a fee of $40 for missed pre-scheduled appointments with no cancellation notice. A patient that repeatedly cancels or does not show for their appointments will not be able to schedule any further appointments for 6 months.
